The "honorable, other-than-honorable, general" refer to what's called "Character of Service". The "Medical" or "Retired to Temporary Disability Retirement List" or "Expired Time in Service" are entered into the "Narrative" section on the DD214. It's simply the reason the soldier is being discharged as has nothing to do with the character of service.
I read DD214s daily. I've never seen a medical discharge that was not honorable, although I'm sure they do exist. But the character of service is not "downgraded" because a guy is medically discharged, that's for sure.
