I just got a copy of my "Medical Board Report"

AF IMT 618, 19951001, V2 (Is this the same as the Medical Board Summary you referred to?)

In Blk 23: Diagnosis and Findings:

Axis I: Occupational Problems
Adjustment disorder with anxiety and depressed mood
Depression NOS, Undifferentiated somatoform disorder
Axis II: Personality Disorder NOS
Axis III: Migraine Headaches
Axis IV: Occupational Stress in the form of imposed PCS
Axis V: Current GAF 70, past year GAF 80

All are marked as YES incurred while entitled to basic pay and NO on Existed Prior to Service.

Do I need to see if I can get the 'Summary Report' also to see the rest?
