    Question Previous CBHCO Wisconsin Soldier.

    Comrades,
I was placed in CBHCO Wisconsin in Sept of 2004 after getting wounded in Iraq. I was seperated in April 0f 2006. I never received any copies of my medical exams from CBHCO even though I requested several times for them. Last I heard CBHCO WIsconsin moved to Rockford, IL. I've had no luck calling the reserve unit I was activated with, the USAR command or any medical command for that matter. The VA wants me to produce records for my VA claim. Does anyone out there know where I can get my medical records that were given to CBHCO? Thank you for your assistance.

Welcome! You might try here: http://www.archives.gov/st-louis/mil...-form-180.html . I would keep trying with the RSC. Best of luck!

As past cadre for CBHCO in WI/IL, I have emailed the PAD-Patient Admin and Discharge NCO for further details on the posted question. I have not heard back from her yet, but do know that for discharged Army Reserve Soldier's, their medical records are sent to HRC. All medical records of discharged National Guard Soldiers, go to their unit or designated state Headquarters for storage. If I hear something in addition to this, I will post again.
