The title pretty much sums up my question. I'm trying to apply for jobs while still in the service. I go sign on my ratings this Friday. How do I obtain my SF-15?

You download the form, fill it out and send your evidence of your disability with it. It was all laid out in another thread. You can wait and use the VA letter, or like somebody else did, send a copy of your VA proposed ratings, separation orders and DD214.

Thanks for the reply. I'm not trying to send my proposed ratings because my eye injury would probably disqualify me from a majority of jobs, and they would be able to see that. Where do I download the form from?

Also, some federal jobs will not take the SF15 with a proposed ratings, so you may have to wait until you receive your final ratings after you get out...
