Dictation

Looking for some guidance and advice. Here is the situation:

I am in my second period of Limdu, and having just PCS'd to Quantico I met with my new Primary Care Manager. I explained to him my history, etc and we agreed that we needed to start the PEB process and he directed me to go see the MED Board Officer / PEBLO. So i started my PEB checklist and got everything done and all i needed was a dictation (aka: narrative summary).

So i met with my PCM today and advised that i needed a dictation/narrative summary. He said it will take some time, in which i understood b/c my case is rather complex. He then leaves, his nurse comes in, finishes what she needed to get done, and I asked for the doc again b/c i had some questions. She said the Doc was already with the next patient - and I was annoyed b/c i had some more concerns/questions. So I gave my questions to the nurse so she could talk to the Doc. I waited in the lobby and she came out and told me to follow up w/ the Doc in 30 days. Oh, and by the way - he is tenativly going to be deployed to Iraq.

Just Great! So i attempt to make an appointment and his schedule isn't available b/c its blocked out for his deployment. I was furious! Not only did the Doc not get a start early in the month with his portion of the PEB process and reviewing my case, i think he intentionally is procrastinating so he can deploy and not have to do my case. I wish he would have told me earlier on that he was due to deploy when we met for the first time and discussed the PEB!

So now i'm stuck in limbo, I don't know if my PCM will be around, or who my next PCM is - and worse is that I'm afraid there will be no continuity with the next doc and I will have to explain my case all over again.

Does anyone have any suggestions on what I should do? I feel like going back to the clinic and confronting him directly or speaking to his boss!
 
Dear Friend,

1. If either of your assumptions is true regarding the PCM not wanting to draft your med board narrative, then it should actually be a relief that he will not be doing it.

2. After all, if he does not want to spend the time to do it well, do you really want him drafting it?

3. My suggestion is very straightforward- begin to organize your personal copy of your medical record by subject specialty, have a list of your symptoms and limitations, medications and other forms of treatment, how your condition(s) do or do not (depending upon your goals) affect you in the workplace.

4. These things happen for a reason- rather than get upset, recognize that it happened to allow you the time to get your ducks in a row.

Thanks for the reply. Ever since my medical problems happened, I have saved and organized every medical document in folders. I have everything ready to go except a dictation. I even explained to the doc i have everything, and even gave him a two page quick synopsis of cause and conditions. That and everything is in electronic medical records as well.

I sent him an email, if i don't get a response, i'll swing by the clinic and try and see him. If not, i'll change PCMs.

Good thing him and I are the same rank :cool:
 
I think that changing PCMs my be the best solution. Congratuations on saving and organizing your medical data. You could probably write your own NARSUM. In any case if and when you prepare a VA claim this or preparation will be a great advantage. Good luck and keep up the positive outlook.

Does it matter who writes my Dictation? Does it have to be my PCM, or can one of the specialists i have been seeing write it?
 
1. In the Navy system, it is typical for the PCM to initiate the board and have specialty addenda drafted by those providing the specialized care for your condition.

2. However, if the circumstances warrant, one of those specialty providers may initiate drafting your board.

3. Frankly, if this person does not appear interested in drafting your board, you do not want him as the drafter.

4. In addition, if he is deploying soon, this will leave someone else to tie together the specialty addenda anyways.
