Generally, I would say, they should focus on the present. However, under DoDI 1332.18, they can (and do) consider whether "(2) The Service member’s disability imposes unreasonable requirements on the military to maintain or protect the Service member." That has been used in many cases to "project" that future worsening of a condition renders a member unfit. The issue is somewhat complicated.
