ALC and referral to DES?

I just heard back that my IRILO was reviewed by NGB/SG and I was RTD with an ALC2 for one condition. The NARSUM addressed other potentially unfitting conditions as well, but the ALC2 only addresses one of the conditions.

Is there a chance that NGB/SG will make a separate determination on the other potentially unfitting conditions, or have they essentially made their determination by not addressing them in the ALC2 and by default determined that the other conditions do not qualify as "unfitting"?

The IRILO was for a host of long-COVID symptoms as well as chronic issues with my ankle and knee which have both been operated on. This whole process has been a struggle with both NGB and my local GMU.

Chances are you have hit the end of the road. Not in a good way. Have you consulted with a private attorney? Might be worth looking into. The NG is notorious for not following their own regulations. Attorney's are pricey but may be worth looking at. I will send you some references via DM in case you wanna explore that.
 
Quick update: I contested the IRILO to my chain of command who then advocated for a sit-down discussion directly with the preparing flight doctor (I'm not a pilot, but my AFSC requires certain medical qualifications). The SGP flight doc hadn't consulted nor evaluated me initially, but he was basically forced by the commander to hear me out. He was actually pretty approachable about the whole situation and agreed to "take another look". This was right before the gov't shutdown last year. Knowing there was little guarantee that I would get the NARSUM re-written, I went back to the active duty flight doc who had written a now outdated initial NARSUM because he was the treating physician. He agreed to do it which was huge. This was October 2025.
In November 2025, my BCMR came back reversing all NILOD conditions to ILOD, which only serves to strengthen my NARSUM and IRILO package making an MEB almost irrefutable. Because of the shenanigans at ANG/NGB, the BCMR also granted to have my new IRILO looked at by AFPC rather than NGB.
I'm now just waiting on the new NARSUM to be finalized. This was a total answer to prayer. But, let this be a word to the wise for Air Guardsmen: NGB makes it exceptionally difficult to get In the Line of Duty consideration, especially if you have multiple conditions. They basically think you're trying to get a "golden ticket" and will deny an LOD, which means if you're potentially unfit for service, then you get shoved into the Non-duty DES. This motive shows up not only in LODs, but also that they'll only selectively review an IRILO and return you to duty - in my case for a condition they previously told me in a NILOD determination that I did not have! NGB is not your friend!
