DA 31 for PTDY fill out.

Hello everyone, I am waiting for my separation letter which is suppose to come soon mean while trying to figure out few things, filling out da 31 the proper way is one of them , I do not want to deal with s1, company COC none sense and I want to do things the right way so my question is:
Do I fill out two separate leave forms? - one for PTDY and one for terminal leave or
Do I fill out a single leave form for both PTDY and terminal leave? And if so, how do I fill it out?

Does any one done that can send me a filled one.
any help is appreciated.
 
It's should all go on one DA31, the leave type will be "transition leave" and then put in the total amount of days and in the comments block u can break it down so there isn't an issue .. so let's say yur taking 20 days ptdy and 10 day regular leave starting tomorrow it would look like this...

PTDY - 20170823 - 20170901
Leave 20170902 - 20170911

There is also a spot where you can write in the "chargeable leave" which in this example would only be 10 days

Thank you, this sound pretty direct and clear however S1 still doesn't want to do it this way thinking there is specific way cover in regs, I have went through the regs and didn't not find any specific guidance.
have you done it this way?
 
I have never seen anything in the regs that shows how to do it a specific way..that comments area is for stuff like that to help the S1 people when submitting your leave into the system .. that is how I did mine and my s1 accepted it.. but S1's can be a real pain in the ass sometimes.. tell them that if they have a specific way they want it have them give u a template because thats BS.. and if they still give u crap just go to the command and let them know that they are bs'ing you..

I am an HR NCO and have ran multiple BN S-1's.

There is no correct way to do the DA31. The people who would care the most is finance and as long as there is no gaps, they could care less.
The 1 leave form vs 2 debate is all up to unit standards and ow commanders want to see things.

You can get advice from 20 different people and you will get 20 different answers. The only thing you need to do is ask your S-1 how they want to leave form or forms so you do them the way they have been instructed to receive them and avoid any delays correcting and resubmitting.

OAN, how can you be doing a leave form if you don't have orders yet which will have your exact retirement or separation date?

Although this is true, that section of the form is not for the requesting SM to fill out. That is for S-1 personnel to compute AFTER the leave is done, which period of time it was chargeable. We on the 1 shop follow multiple regs and policies that an everyday Joe may not be aware of when charging leave comes to play.

For example, in some cases if a leave period ends in a Sunday, the SM will only be charged until Saturday, among others.

But hey, if you want to get extra charged, by all means fill that out, less work for us to compute Lol

Yes I went back to S1 and had it figured out.
I am at the same step as you are except my last date was the 17 not the 24.. I am just getting everything ready so when I get my LOR I can submit everything in order to get orders, the lady is very helpful and she doesn't need signed leave form , a draft one is fine.
