HELP! DA 7652 Commanders Functional Statement HELP!!!!

My commander has asked me to fill out the DA 7652 Commanders Performance and Functional Statement since he is new and hasn't observed me. I realize this is a great opportunity but I really don't know how to word a few of the sections.

I am being recommended to a MEB for PTSD and have all the usual symptoms which prohibit me from working full days and I am currently assigned no duties in my MOS. I also have neck issues, DDD, that will assist in the MEB but this form is specifically for PTSD/TBI.

Does anyone have an example DA 7652 they would care to share with me or any input that will make the form strong and effective.

This is a great opportunity!

Generally, you want to talk about how your job performance has steadily been worsening or has been chronically bad since your conditions have came about. You most definitely want to point out the fact that you are unable to do ANY tasks or duties with in your primary MOS.

What is your MOS?

I think that any commander's letter (in the Army, the DA 7652, in the Navy, the NMA), is strongest when it addresses EACH Condition (or symptoms that are obviously related to a condition) and SPECIFIC examples of duties that cannot be performed or are significantly impacted.

BAD (but common) example: "Due to SGT Smith's conditions he cannot deploy, work in an austere environment and is unable to perform the duties required of his grade and MOS."
This is really of little help to the PEB and if there is more than one condition, sets up a situation where the PEB may rate only what they perceive as the "worst" condition.

GOOD example: "SGT's Smith's asthma prevents him from performing the APFT and from any significant exertion. As a Wheeled Vehicle Mechanic, his asthma prevents him from working in the motor pool, due to the fumes common in maintenance operations. I had to remove him from working in the motor pool as he was having severe asthma attacks due to the environment. His back pain prevents him from moving in and around vehicles and makes him therefore unable to perform maintenance. He also is unable to even a small tool kit (25 lbs.) due to his back condition and this makes him unusable as a mechanic."

The terminology may change between branches, jobs and ranks. However, the concepts are similar. Specifics as to all conditions and their impact on duty performance is key to providing the PEB with the information necessary to properly rate all condition.

I've been given the opportunity to write my commanders letter and it is due today. Can you give me some advice or review what I've written so far and let me know if its strong enough. I'm looking to be medically retired at 80% or more and be determined un-employable due to my conditions. Please help.
 
Personally, I'd write down my own observations on a separate piece of paper and tell the Commander to do his fu***** job. The entire core of the military is eroding away with shit leaders like this.

I am sure this will work well? What he will get is an extremely late document that is written exactly like Jason's example of "bad". Having spent 13 years in myself as an NCO and Warrant Officer, I can tell you I always took the lead on documents that affect my life. I always wrote a draft NCOER, OER and Award. I would then turn that over to the rater. Yes it's there job, but it's your life.

Seriously? I'm not saying give some incompetent commander the keys to my life, by no means. How wouldn't it work well? Not saying be disrespectful, but to let him know, with tact that he is required BY LAW to fill the form out.
 
What if Commander is BS. They say you do eveything fine and no problems even though you don't?

I.G. Complaint, your Commander isn't a medical professional, so let your medical records/injury speak for themselves.

Or you can let these people to continue to abuse their power and you can live in "fear" of conforming to Regs/Policy. I wish a Commander would "black ball" me for holding them to the standard. Most people just become sheep and regardless of how wrong something is, it's what they are use to so it makes it ok I guess. So sad.

oh, and do you think an Officer is going to put their signature on a legal document that they lie on cause they are "out to get you"?

Yes, seriously! Yes he is required by law to do it, but not to put any effort into it. Things can be well written and things can be written to the minimum required to get by. Tact will get you a long ways, but any time you remind somebody they have to do something according to law, is going to run you into a wall. Maybe in some cases, it might work, but overall, you are killing yourself.

Why is it an issue to write a draft document and propose it? Is that not in your best interest? He then has a choice to use it, edit it, or write his own. Either way, you have the wheels turning and have a document in front of them that gives them ideas. That to me is the way to tactfully do things.

What exactly do you feel the Op gains by going in and politely reminding the commander he has a legal obligation to write a commanders statement?

If you read my 2nd post, I actually suggested writing your own wording to give the Commander. My reply is mainly to the people who said their Commanders said "do it yourself"

Can someone help me out with a fillable DA Form 7652? A blank fiorm or any coments related to PTSD, mTBI, depression, anxiety, neck and back injuries, bilateral knees and bilateral shoulder arthritis. Any help is appreciated, thanks. Can also email to [email protected], thanks much.
 
If your Commander gives you the option to fill it out, do it. My Commander was much more worried about his retirement party and what the command could laud him with to even care that I was in IDES. When I filled out the paperwork for him, he said "this is the first that I've heard of this" - well duh, don't talk to Soldiers and you don't know anything. But he's gone now and the new Commander seems to at least want to know who is in the unit and if we have problems. The bad Commander's Statement did appear in my NARSUM making getting a fair rating that much harder.
 
My CO called me to her office and sat down with me and asked me if I wanted to stay in or get out... She wasn't going to leave what I wrote in the NMA unless I actually wanted to get out. I still think she beefed it up a bit after that because mine was somewhere between Jason's examples.
 
my question is where on this form do you put anything other than mental health? section III B is for mental conditions and Section III C is for TBI. I dont see any other sections to input physical problems... this form makes no sense.
