The majority of the documents submitted in my husbands LOD investigation are inaccurate. I am seeking truthful opinion for DA2173-Statement of Medical Examination and Duty Status.
The concern I have is in block 6- dead on arrival is checked and he was not DOA. Block 7- the name of hospital or treatment facility listed is not the hospital he was treated at immediately or ever. Block 17-Name of attending Physician or Patient Administrator is signed by a Physician at the hospital listed, not the one he was treated at. The date the document is signed approximately 9 months prior to VA noting they received it. VA notes documents were received over a 9 month period.
I'm being told that the information is accurate for what the form needs to report.
This seems significant since documents submitted are inaccurate and each document is meant to objectively present the incident, which it cannot do if they are incorrect.
it seems as though an inadequate investigation was done with multiple documents being inaccurate, these vary from inaccurately identifying markings on his body to when he was transported to the medical examiner's office, dated prior to his death.
