LOD

Active Army-I have some questions and hope they can be answered. I was injured in April 09 my Unit didn't do a LOD now my packet has gotten sent back from the PEB and they have requested one, How do I go about getting an LOD 14 months after the accident? I am not in the same unit I have been sent to a WTU. I have asked my Squad Leader he said he couldn't initiate one, so I turned to my NCM, any advice would be appreciated. My MEB is 16 months in and just want to get this locked in with the right information when it goes back to San Antonio.

One thing I forgot, My Insurance Company has already paid back Tri-Care for all of the treatment from the accident and I am in the process of a claim against the person who hit me, I have turned all of this information in with my packet to go back to The PEB. What else will I need and will I still need the LOD with all of this info? I also have statements from my Attorney that will be going with the Packet on the return trip.
