Can anyone provide information on what needs to be included in a PLD package? The references are pretty useless in this matter. templates or examples would be very helpful.
 
Have you looked on the MyNavy HR page for PLD yet? You say the references are pretty useless, but you don't mention what you have looked at so far.
 
Yes I have looked at that section. It says to refer to SECNAVINST 1850.4(series). The only mention of PLD in there is in Enclosure (2) under "Duties and Responsibilities" of CHNAVPERS/Deputy CMC, Manpower and Reserve Affairs. The best information that I have found is in SECNAV M-1850.1 Pg 4-31 to 4-34. On Pg 4-33 halfway down, section (4) states "...must submit a request, with command endorsement, to...." Given that information, is it safe to assume that a request in official memo format routed thru the CO will suffice?

Tracing back thru "references" of references, there is mention of PLD or the Definition of it but nothing towards a "procedure" or "Package contents". MILPERSMAN 1850-020, MILPERSMAN 1300-1400, DoDI 1332.18, 10 U.S.C 1200 through 1400 sections are just a few of the list of references that I have reviewed.

When my IDES first started in 2021 during the initial trainings I recall PEBLO showing a specific form for requesting PLD and acting as though it was pretty much a given if one is over 18 yrs. Now the PEBLO's are nonresponsive to any communication after sending me "Finalized" findings the last business day before Christmas, starting the 15 day clock.

I have even reached out directly to PERS 454 (where the package is to be sent to by COB on 15th day) via phone and email, but have not gotten a response yet.
 
Ok, sounds like you've done some pretty exhaustive research. Kinda surprised there aren't any templates on the MPMs or MyNavyHR site, that's usually where something like that should reside.

Hate to tell you, PLD isn't guaranteed at all, and anything over 12 months is *definitely* hard to come by. There have been a lot of stories about it. You might get more response on this topic on the main FB forum - I've seen a lot of discussion on Navy PLD on there. Search "Military Physical Evaluation Board Info" to find it, then use the search function.

Yes, I agree. The timing sucks but is what it is. Trying to get something routed thru the CO with two 4 day holidays inside the 15 day timeline is nearly impossible.

I know it is not guaranteed. And looking at some of the prior revisions of references it's pretty obvious they are trying to minimize and all but do away with PLD. Given the current date on the retirement msg that MSO/personnel received, I only need 4 mo to put me past 20 (currently at 19y 5mo). Being this close, I have to at least try.

If you only need a few months, why not just reject IPEB findings and request FPEB?
Have already went that route. FPEB was in October and then VARR followed. Received those findings and "Case Finalized" on Dec 22nd. Exhausted every avenue available to buy time thru ought the process. Even requested an extension, but was denied, due to inability to get any medical appts in less than 2.5-3 mo out.

I'm not an expert on this, but I thought there was an appeal option to FPEB, taking it to the CORB. No?

There is an appellate review request still allowed by the Navy. From what understand, it is just a paper written up and submitted by one's attorney that goes to a higher review and is either accepted or denied. Other than buying extra time, my case would not have been a candidate for that. However, I was not able to use that because I accepted a pre offer from FPEB the morning of the hearing (did not know this at time of accepting) The offer contained more than what the attorney was expecting me to win. I guess there's always trade-off's for everything.
