To who it may concern,

Recently received a new diagnosis and then COVID19 hit. It was very devastating disaster that swept the nation killing hundreds and thousands of people nation wide. As for me it affected my Behavioral Health Counselor and she fell ill to COVID19. When this happened, my profile was need sent in to be revised instead the profile stayed the same stated the same things. After, my appeal was sent in, it came saying that I was not fit for duty because the profile stayed the same. I don't know what to do please help me?
 
Talk to your MH providers supervisor. He/she should be able to see the record and make appropraite changes.
 
I have two different BH offices I went to see.. one is the main one and the other was where I received my last diagnosis.. my question is do I go back to the main one and ask for my Nurse case manager to revise my profile since I have a new diagnosis?
