Question about 4138

I had one of my C&P exams recently and in the DBQ they under-diagnosed me. I reached out to my MSC and she told me to fill out a 21-4138 to write the corrections. I have not received any form of rating since the rest of my exams have not been scheduled.

Questions:

1. Can I put medical records that show evidence that the diagnosis is wrong?
2. Is this the same form used for coworker, supervisor, spouse claim support statements?
3. Do I have to go straight to VARR or will this suffice since I haven't been given a rating?
4. Would I need, or Can I get, the C&P exam redone?

You can file a VARR for ratings related to unfitting conditions. For fitting conditions you must appeal.

Some people have been successful in get a C&P redo. Take to the contractor that provided the C&P examiner and your MSC.

PTSD to adjustment disorder. I reached out to my msc and was told to fill out a 4138 with details and corrections. When I spoke to the contractor they told me I would have to do a varr through the VA
 
MH C&P examiners provide an independent diagnosis. Do you have documentation of a differential diagnosis in your records? My experience is thatMH diagnosis is incorreact all to often.

Yes I called the contractor and they told me the MH provider used old records because that's what was submitted with the package to the VA initially. I am in the process of obtaining all my mental health records to submit with statements from coworkers, and supervisors.
 
I suspect the correct records alone will suffice.
 
Unless the PTSD is combat this is kind of unnecessary all MH orders are rated the same based off of social and occupational impairment.
