Jan 21: Applied for regular (20.7 yrs) Air Force retirement, and got approved, received retirement orders.
3 Feb 21: Completed SHPE
23 Feb 21: Contacted by SHPE POC, she said since I had chronic medical conditions I was being referred to the Deployment Availability Working Group (DAWG). I was upset even though I was retiring due to me medical issues; however, I was not looking forward to the potential delay. As I struggle everyday to do the simplest tasks.
Dx dates:
25 Jun 17: Anxiety/Panic Disorder/Sleep issue(insomnia)
**Aug 17: PCS’d**
17 Apr 20: Meniere’s Disease
21 May 20: PTSD
**Jun 20: PCS’d and started med care at an Army base, they don’t have access to AF profiling system**
13 Aug 20: Chronic Migraine treated (w/Botox automatic MEB for AF)
21 Jan 21: Chronic Fatigue
10 Mar 21: 2nd opinion/Confirmed Meniere’s
13 Mar 21: DAWG
5 Apr 21: AF 469
8 Apr 21: 1st PEBLO contact
9 Apr 21: Returned Intake to PEBLO
: PEBLO response, sending to AFPC.
