CRSC DD 2860 Fillable Questions

I had a quick question regarding filling out DD FORM 2860. Talking with a VA Rep about the CRSC form he mentioned that I need to provide justifications and explanations for each condition I am claiming CRSC for. For a very short background, I was put on TDRL at 70% with a VA Rating of 100% P&T. I understand that the VA is different than DoD, however the 2860 requested my rating sheet which I have.

For my CRSC application I will be listing, PTSD/MDD/AUD/TBI as the primary. For my VA paperwork and code sheet it doesn't say anything specifically as secondary however I would like to add Post-traumatic headaches, Astham, IBS, and GERD. I was considering adding rotator cuff tendonitis, cervical strain of the intervertebral disc, encroachment of the C3 to C5 foramina in dominant arm, right arm and hand tendinitis with loss of use, and Tinnitus. For the neck encroachment I can justify it was tied to the TBI event and then caused nerve damage due to compressing the nerves going down to my dominant arm/hand.

My questions are:
1) For the Title of Disability, I assume I list the PRIMARY condition and body part affected?
2) For the Prisoner of War block 11 - I have appropriate documentation covering this event however was not "awarded" if you call that an award, the POW Ribbon for it - does that make a difference? I was going to select it and provide the medical and Command documentation, but wasn't sure how it was viewed and honestly don't even like checking it or getting into it as far as the details.
3) For block I, Many of the secondary CRSC claims I'm submitted happened over 6-7 combat deployments, so I just listed all my deployed locations, does that sound correct as block I is written in singular vice plural form.
4) For block M. it specifically says following conditions are secondary... If they aren't established as secondary by the VA, but I want to put in for CRSC and have the appropriate documentation do I just list them in this section anyway?
5) With the primary, I understand breaking down what happened in section J. Do I also add my secondary / additional claimed disabilities as well or should I attach a signed word document or memo explaining each condition being claimed, or do I need to submit seven or eight different DD2860 at the same time?
6) The VA rep mentioned that based on a SOF background, I did would not need to submit my evals / awards etc as they're listed on my DD214 and my MEB/PEB documentation should suffice, however that does not cover some of the other conditions I'm submitting for CRSC. I will be submitting my Burn Pit memo from the JTF covering all that fun stuff. Should I submit my evals / awards anyway as they include the narrative portion of what happened and per DoD are considered official documentation to substantiate the other conditions?

Hey, Venom, sorry for the delay:

1) Yes indeed
2) I would check it and include your documentation that you have, let them decide yay or nay
3) You can go with your last deployment, they're not picky ;)
4) So, if they're NOT listed as "SECONDARY" by the VA, leave blank, for additional conditions, if you have the most current version of the form (it will have "CUI when filled in" at the top - I'm attaching just in case, save it to your local device, then open in Adobe Reader, otherwise you'll see an error), you should be able to click "Duplicate" at the bottom of the page, then insert the number sheet of sheets in the bottom right-hand corner (i.e. Page 2 - Sheet 1 of 5/2 of 5/etc). So, to summarize, make sure you go through your VA conditions and determine which one(s) you can tie to a combat-related event(s).
5) See #4 above, and it's just an additional Page 2, not a complete DD-2860
6) If anything, you won't need your evals, but I would certainly include your combat-related awards
