MED/PEB BOARD Initial submittal Questions and how to be screened for DOD Disability

I currently have two issues going on with me. I recently lost a good amount of vision in one eye, and the cardiologist said that his best guess is that a congenital heart defect (Found this year after 11 years of service and had to be waivered at NAMI to keep doing my job as well as shows that military service has aggravated the condition) could have possibly played a role in it.

My PCM is about to refer me to Med board for the eyesight. When it comes down to the PEB board and DOD disability findings, Do I also need to have the have the heart defect referred with the eyesight for the initial MED board in order to be looked at for DOD disability for it? The fact that I am waivered but now potentially showing signs of it affecting me doing my job feels makes me feel as if that needs to be in the claim.
I understand the disability will be looked at under the VA and given a percentage for it, but do i need to have that initially referred in the med board in order to get a dod percentage? Or does the DOD look at every possible thing wrong with you then determine if those affect your ability to deploy and/or do your MOS?
