Question on 526EZ Form

I'm pretty sure I am nuking this but on section V for the claim information, How specific does each claim have to be? For example Joint stiffness do I list left knee and right knee separately or can I just put joint pains? Second for the How disability relates to service, using an example like Crohn's disease, how do you specify a relation? I am still active duty, can I just put Active service?

This form really makes me feel dumb and haven't received a response from the PEBLO or the attorney and only have a day to finish it. You all have such great info!

Section VIII of the form provides a place for addendum info. This can be used for nexus (relation of disability to the service) information among other things.
